Change Request Form
Agencies are being asked by CSD to use theCSD Request for Client Name/SSN Change Request form when submitting CORE Data Change Requests to the CSD Energy Help Desk.
ServTraq© has a built in form that automates the process of requesting a Name/SSN change in CORE while preserving an electronic record in the database.
The ServTraq © “CSD Data Change Requests” allows multiple requests to be logged and submitted to CSD in one batch. This form has been a feature in ServTraq since the launch of the software, but some users may have missed it.
The CSD Energy Help Desk will continue to accept the form provided by ServTraq as an equivalent to their form. The form can be found under Reports>Daily Reports> CSD Data Change Request. Be sure to set up agency phone and fax numbers to receive a quicker response from CSD.
Agency Contact Information can be added under Administration > AgencyDetails
You can now populate Individual Household Member information from the most recent Payment Assistance Application record onto the current Payment Assistance Application being entered. If you use the “Individual Household Members (CSBG Data)” option to collect household information, simply click on theImport Household Memberbutton to populate the demographic information. If the household composition has changed you can add, edit or delete entries copied over.
